Community Health Center
HopeHealth is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n). We became a Federally Qualified Health Center in 2007. This designation allows us to provide essential medical services to the uninsured, as well as those with Medicaid, Medicare and all other private insurance.
A community health center functions like any other primary medical care office in many ways: We employ skilled, licensed physicians and nurse practitioners to provide primary and preventive health care to patients of all ages. What sets us apart, however, is our commitment to providing accessible care to all patients, regardless of insurance status or ability to pay. We happily accept all forms of insurance including Medicaid and Medicare. We also work closely with patients who lack adequate health insurance to help them qualify for our sliding fee discount program.
Infectious Disease Services
Our commitment to HIV treatment begins with prevention. We provide free walk-in Rapid HIV and Hepatitis C (HCV) testing at our sites and also go out into the community to provide free testing to the public. These tests are confidential, with results in 20 minutes including pre/post test counseling.
Other prevention efforts at HopeHealth include community education and outreach to at-risk populations in our service areas.
Intake and Referrals
For those who have received an HIV-positive diagnosis, we strive to provide a patient intake process that addresses not only the physical health concerns of the newly diagnosed, but also the emotional, spiritual and financial health of our clients. No one should have to live with or cope with HIV alone, or without adequate information about managing this disease. Our medical case management staff connect their clients to essential services and coordinate medical appointments, facilitate referrals, linkage to care, transportation, and intake procedures for various programs. We also coordinate mental health care and support groups to provide compassion, encouragement and empathy to our patients and clients.
