No insurance? Not a problem. If you are uninsured, LRMC offers programs that allow uninsured individuals and families to receive services for a fee that is based on household size and income. Our eligibility staff can determine if you or your family qualifies for Medicaid, CHIP or other local, state and federal programs. So that we can best assist you, we ask that you bring the following information to your first appointment:
Proof of income for everyone in household. This can be recent pay stubs or tax return statements.
Confirmation letter for any other income (SNAP, SSI, disability, VA benefits, etc.)
Proof of residency. This can be a recent utility bill, lease or county tax receipt.
Photo ID card
Social Security card
Any evidence of insurance coverage.

In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
