Our Sliding Fee Program offers reduced prices on office visits and services provided in the clinic to all individuals and families who qualify. If you apply and qualify at one Horizon location, you receive the discount at all of our medical and dental clinic locations.
The Sliding Fee Program is based on family income and may reduce your bill 20%, 40%, 60%, 80% or more. You may qualify with or without insurance and you may qualify even though you have Medicare or Medicaid. To see if you are eligible, find your family size on the table below. If your family's annual income is less than the amount listed, you qualify.
To apply, submit the following: a copy of the front page of your most recent income tax return or a copy of your most recent pay-stub if you didn't file an income tax return. Also, you must submit a completed Sliding Fee Program Application. Applications are available at any of our locations - just inquire at the front desk

In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
