Services at this location include:

  • Medical services: primary care, well-child visits/developmental screening, immunizations, school/sports physicals, and pediatric specialty care.
  • Ob/Gyn - Women's Health Services: birth control, family planning, well woman exams, pap smears, prenatal care, etc.
  • Psychiatry: mental health assessment and referral.
  • Dental: cleanings, preventive care, restorative care, fillings and crowns, root canals, tooth removal, infections, retainers, braces, scaling, root planning, dentures and bridges.

CHN accepts most private insurances, Medicaid, Medicare, and CHIP. Uninsured individuals may be eligible for discounted self-pay services based on their family size and household income.

CHN does not turn away anyone away due to an inability to pay.

Our mission is to provide quality care that uplifts, supports, and shapes healthy communities while addressing financial and social barriers.

For Households with more than 8 persons, add $4,480 for each additional person.
*Alaska and Hawaii have different rates for HUD federal poverty guidelines.
These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
