The Live Oak Community Health Center is a satellite clinic of the Atascosa Community Health Center, Inc. Opened in October, 2012, LOCHC is conveniently located and serves the residents of Live Oak County. Our providers and staff are knowledgeable of community resources and make referrals when necessary. We work hard to provide all of our patients with the care they require in a comfortable environment.
Atascosa Health Center, Inc. accepts most major insurance policies, including Medicaid/CHIP, and Medicare (varying by clinic). If your insurance is not found below, contact our clinic and our staff will work with you and your insurance provider to provide you with the care you need.
*We recommend you check with your individual health plan to determine coverage at any of our facilities. You are responsible for any co-payments and deductibles depending on your insurance coverage. At this time, we do not accept any worker's compensation.

These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
