SPCAA PHC Project
The SPCAA Primary Health Care (PHC) Project is a co-payment program in Dickens, Crosby and Garza Counties for outpatient medical care at contracted medical clinics with minimal cost to eligible clients.
All enrolled PHC clents are eligible for the following:
Health Risk Assessment and counseling
Baseline History and Physical Examination
Out-patient Clinic Appointments
Out-patient Laboratory Tests and X-rays
Limited pharmacy assistance with prior approval
Chronic Disease Case management
ALL Dickens, Crosby and Garza County residents are eligible for the following:
Benefits Screening and Referral
Eligibility Determination
Persons with any kind of private medical insurance, Medicare or Medicaid are NOT eligible for PHC enrollment
Medicare or insurance co-payment are NOT paid for by the PHC Program
Hospital In-patient and Emergency Room services are NOT paid for by the PHC Program
