This facility offers a sliding scale discount. Those clients who fall below 100% federal poverty level income, a minimum user fee of $15 per office visit will be required in addition to any applicable fee(s) for any ancillary service(s) provided. Medicare clients, the Center will waive the $200 deductible and 20% co-pay for office visits. Medicare clients will be required to pre-pay for medications at the rates mentioned on the previous page.
