Patients with Insurance
MEHOP Health Center is an in-network provider for Medicare, Medicaid (including CHIPS, Star and Star Plus) and many Private Insurance carriers. If a patient has insurance, their carrier will be billed according to services provided. Applicable co-pays and deductibles will apply. Benefits will be verified, so please remember to bring your insurance card to all appointments.
Patients without insurance or have limited insurance benefits
If you are uninsured or have limited benefits, MEHOP offers a sliding fee scale (discounts) to help with costs involved in receiving healthcare.
Please schedule an appointment with our Eligibility Department prior to scheduling an appointment with one of our providers to establish placement on our sliding fee scale, which is based on household income and family size. For example, if a family of four earns $44, 700 or less in annual household income they would qualify for one of our sliding fee scales.
During your appointment with eligibility, we will require the following current information in order to ensure proper placement on our sliding fee scale. The appointment with eligibility takes approximately 15 minutes. Once established, eligibility is valid for 6 months and will need to be updated after time period expires.
Proof of income for household (examples below)
Current Tax Return, 3 Pay Stubs, Unemployment verification, Social Security Award Letter, SNAP verification, etc.
Utility bill for address verification
Verification of those living in household
Insurance Card ( if you have insurance)
Since this is a sliding fee scale clinic, we have provided the Federal Poverty Guidelines below. Visit the Matagorda Episcopal Health Outreach Program website listed above to see what the level is needed for free care.
