This site has a dedicated staff that is ready to provide quality health care services, regardless of one's ability to pay. This site accepts most private insurance plans, Medicare, Medicaid and CHIP. For individuals at this site who are not covered by health insurance, CDHI has a sliding fee scale based on the client's family income and family size. This site is in accordance with Federal regulations, and proof of income must be provided for the client to be eligible for reduced fees for services and prescription medications. This site charges a minimum $18 for medical services and $30 for dental services prior to treatment for those not covered by insurance. For additional charges, payment is due upon receipt of services at this site.
