Must register at the clinic
Must be a Franklin County Resident
Income at or below 200% of the federal poverty guidelines
Single Person - up to $22,980 a year
Couple - up to $31,020 a year
Family of 4 - up to $47,100 a year - higher income may be had with more family members

Bring the following information to your registration appointment:

- 4 most recent pay stubs
- Unemployment benefits letter

YOU MUST REGISTER AHEAD OF TIME TO SEE A PHYSICIAN.
Call the clinic at 540-489-7500 between 8:00 A.M. and 4:00 P.M. Monday through Thursday for an appointment to register. You will be asked to bring the following items that apply to you. If you bring all required information to register, approval for services can be provided the same day as registered.
