We offer a Sliding Fee program for all persons that are uninsured.
What is the Sliding Fee program and how do I qualify?
Sliding Fee program allows you to receive healthcare services at a reduced rate know as a copay. The copay is based on household income and the number dependents supported by the household income. You will need to bring in proof of income and dependent name, age, and social security number.
How much do I pay if I do not qualify for the Sliding Fee program?
The copay will be $140 for anyone that does not qualify for the Sliding Fee program.
Do I have to pay at the time of service?
Yes, all payments must be paid at the time of service. This includes insurance and sliding fee copays.
