To provide medical care and medications to indigents.
Open from 4 to 7 p.m. Tuesdays and Thursdays
for new patients to do an
intake and see if they qualify for the services of the clinic. The Clinic also provides limited dental and eye care. Volunteers provide almost all of services.
Eligibility:
- You must be at least 18 years old and a resident of Madison County as shown by a driver's license, utility bill, pay stub or other legal document.
- You must be uninsured for medical expenses. This includes Medicare and Medicaid. However, if you have Medicare and Medicaid you may still be eligible for prescription assistance and limited dental and vision care.
- Your total family income must be within the federal guidelines. You will need documentation verifying all sources of household income.
- You must have a recent federal tax return, if applicable.
The primary health care comes with a $3 charge. Generic medications through Madison Drug Store are $1 for each prescription. There also is a pharmacy assistance program for drugs that are not generic. For example, insulin is a non-generic medication.

For Households with more than 8 persons, add $4,480 for each additional person.

These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
