Service description:Community health clinic provides physical health exams, acute and chronic health care, immunizations, physical therapy, care of chronic conditions, dietary health counseling, women's health screenings, anonymous HIV testing and counseling, limited mental health counseling by appointment only, chiropractic, lab and blood work, x-rays, prescription assistance program, and health care case management. The clinic also provides referrals to other health and non-health services, and will screen patients and callers for eligibility for other health services and public insurance programs, as well as assisting with the application process.
Eligibility:Persons without health insurance or who are underinsured with income at or below 300% of the Federal Poverty Guidelines
Service hours:Middlebury: Tuesday and some Thursday evenings 6:00pm-9:00pm by appointment only; Vergennes: Every other Thursday evening 6:00pm-9:00pm by appointment only
