Service description:Community health clinic provides a one-time free clinic, medication assistance, and a continuing care program for 90 days while patients apply for Vermont State health insurance programs. Services covered include treatment for illness or injury, routine checkups with a primary care physician, annual exam, prostate exam, mammograms, lab tests, immunizations, health education classes and help applying for insurance coverage. The Continuing Care Program will provide services for a renewable 90-day period while a patient is obtaining health care insurance.

Eligibility:Income eligible community members who have no insurance, or are under-insured.

Relevant notes:Continuing Care Program can be renewed if patient is unable to receive coverage.
