Spine Clinic of Madison is the only health care clinic offering free Chiropractic health care services in the State of Wisconsin. We also offer a sliding fee schedule for patients not meeting the minimum qualification standards for free care. Please refer to the 2012 Federal Poverty Guidelines to see if you qualify. We require that you provide us with a copy of federal income tax return filed last year as well as a copy of all W-2 forms stating income earned in the past year. You may fax or email this information prior to your first visit to facilitate approval.

Spine Clinic of Madison is a for profit small business that does not receive any federal or state funding for our Free Clinic. 100% of the funding is assumed by our doctors and staff. Therefore, we are currently only able to see a limited number of patients. We sincerely hope that we will find the funding to transition to a Free Clinic full time. We have developed ergonomic support products to provide the necessary funding to achieve that goal. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
