We are not a free clinic. We provide healthcare services for a fee. However, because we are a Federally Qualified Health Center, we are able to offer reduced rates to people without insurance as long as they qualify based on income. Everyone is welcome as a patient.
PRESUMPTIVE ELIGIBILITY
Presumptive Eligibility (PE) will be offered to all uninsured patients who may qualify for
insurance coverage.
One of Heart City Health Centers PFS or Managed Care representatives will assist you in
completing the application during your visit.
PE will provide you with instant insurance coverage. This may allow you to be seen at
Heart City Health at no out-of-pocket cost for your visit and affords you the benefits of
insurance coverage. Including;
- Specialty care
- Hospital care
- Emergency care
- Access to prescription medication
During PE, you will need to complete the full insurance application process and submit
your application to the State of Indiana for approval. Assistance in the application process
can be provided by one of the certified navigators at Heart City Health Center. You will
need to make an appointment.
- If the application process is completed and it is determined that you qualify for
Medicaid, you can continue to receive the previously mentioned benefits.
- If you do not move forward with the application process during the PE period, you
will NOT be eligible for Presumptive Eligibility for another 6 months.

Primary Care and OB/GYN Medical Services for Women
Since its establishment in 1993, Heart City Health has focused on treating all aspects of its patients. By bringing primary care and OB/GYN services into one building, Heart City Health is able to provide comprehensive healthcare to women from all walks of life.
Primary Care
Wellness checkups, immunizations, and other general visits with a physician are considered primary care. Our primary care physicians are especially passionate about and competent in working with women. Plus, because the women's health office has other women's health services, patients can receive all their women's healthcare in one convenient location.
Gynecology
From annual pap smears to birth control consultations, gynecology is a vital field concentrated on women's reproductive health. Your gynecologist will be there to guide you through all areas of female reproductive health like menstruation, hormone imbalance, STIs, and complications with fertility.
Obstetrics
Obstetricians take care of pregnant women during pregnancy and after childbirth. By utilizing Heart City Health's women's office, you can have your primary care physician, your gynecologist, and your obstetrician located in one building. That means most of your medical records will be accessible to your obstetrician during visits. This collaboration between medical providers ensures all our patients receive all-encompassing care with their medical history in mind.

Lab services.
Joint injections.
Skin tag and wart removals.
Ultrasound services for kidneys, bladder, liver, gallbladder, spleen, pancreas, bile ducts, and the carotid artery in women and men. Prostate ultrasounds for men are available as well as services for pregnant women.
Birth control IUD.
Colposcopy.
Endometrial biopsy.
Cryotherapy.
Loop Electrosurgical Excision Procedure (LEEP).

These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
